1. Install the Push Health App in the App store or click the "Click to Download" button in green below which will take you to Push Health App
2. Scroll down on the homepage of Push Health App. Locate the "For Patients" section, then press "Learn More"
3. Enter Code: LCORDOVA (all capital letters) where it asks to enter the code.
4. Complete your profile and medical information accurately and in full
5. Luis Carlos Cordova FNP-C will give you a call and conduct a consultation.
6. Your prescription will be sent to your pharmacy of choice!
